All entries must provide a package mailing address using the form below.
Only one entry per household.
The prize will be mailed, certified and insured, to the name first provided upon
entering via the form below and a signature will be required upon delivery. For security reasons,
I can't send it "care of" or to any other address but the one initially
provided. Requests to do so may result in forfeiture of the prize. In that event,
a new winner will be drawn.
In order to claim the prize, you must remain a member of the newsletter list
until the contest's conclusion. The winner's name (first name and initial only)
will be posted in the newsletter by way of notification. Failure to respond will
result in forfeiture of the prize. Bounced emails and unsubscribe requests will
result in removal of name from contest list.
